Report
Report your injury in writing to your supervisor IMMEDIATELY. Make sure you keep a copy for yourself. You only have one year to report most work injuries.

Ask your supervisor where you should go for treatment. If they don’t know, go to the closest Urgent Care, your primary care physician or an emergency room.

Tell the medical professional you see that you were injured at work. Do not leave any body part out. Remember, some symptoms arise days after an accident.

Get a work restriction note from your medical provider and provide to your supervisor immediately.
